SERVICES AND/OR SUPPLIES. Contractor shall provide State the following Telepsychiatry Services at all State Facilities mutually agreed upon by both parties:
SERVICES AND/OR SUPPLIES. Contractor shall provide the Department the following CONTRACTOR agrees to provide temporary nursing services by nurses appropriately qualified, trained and licensed by the State of Montana. Services will be provided at Montana State Prison (MSP) in Deer Lodge Montana, Montana Women’s Prison (MWP) in Billings, Montana, Riverside Correctional Facility (RCF) in Boulder, Montana, and Pine Hills Correctional Facility (PHCF) in Miles City, Montana. CONTRACTOR will provide the Contract liaison with resumes and other pertinent information for each nurse. DEPARTMENT reserves the right to reject proposed nurses, without cause, or to ask for removal of a nurse at any time. The CONTRACTOR shall ensure that all the CONTRACTOR’S Employees sent to the facility under this agreement are: 1. appropriately licensed in the State; 2. have received appropriate training with respect to life and safety issues, HIPPA regulations, Infection Control and the transmission of blood borne pathogens; 3. have current TB testing; and 4. are trained, experienced and qualified to perform the duties specified and made known to the CONTRACTOR by the DEPARTMENT. Further the parties agree that the contractual time period specified shall not be construed as an absolute right or guarantee of any nurse to work within the facility during this time period. Health care services shall include, but not be limited to the following: i. Intake screening/physical assessment; ii. Review of inmate health care record within 24 hours of the inmate’s arrival; iii. Pre-operative care; iv. Post-operative care; v. Short term nursing care for acute illness; vi. Daily triage of Health Care Requests (HCR); vii. Assessment of inmates in segregation for suitability in such placements; viii. Management of communicable diseases and infection control; ix. Provision of routine health care assessment and sick call; x. Maintaining current medical health care records of each inmate; xi. Medication administration; xii. Provision of all provider orders for medication; xiii. Health education; xiv. Emergency care;
SERVICES AND/OR SUPPLIES. Contractor shall provide State respiratory equipment, supplies, CPAP/BiPAP equipment and oxygen cylinders for the Montana State Prison (MSP) located at ▇▇▇ ▇▇▇▇▇▇ ▇▇▇▇ ▇▇., ▇▇▇▇ ▇▇▇▇▇, ▇▇▇▇▇▇▇ ▇▇▇▇▇.
SERVICES AND/OR SUPPLIES. Contractor shall provide the Department the following: Contractor shall pump three grease tanks including two main tanks and one overflow tank up to 4,000 gallons of grease, four times per year. Grease tanks are located at the south end of the receiving gate of the Dining Facility back entrance at the Montana Women’s Prison (MWP) located in Billings, MT. Contractor will also clean the main line once per year by jetting and/or running cable from the grease tank lines to the main line. The Department will maintain safe access to the lines and will not require Contractor to enter manholes or confined spaces. Services will be provided in accordance with the bid response submitted to Invitation for Bid COR-IFB-2018-0102.
SERVICES AND/OR SUPPLIES. 2.1. Contractor shall provide the State with the services described in Attachment A, attached hereto, and incorporated herein by reference. 2.2. Within 30 days after the end of each quarter during which the services required by Contract are performed, Contractor shall submit to the State a written progress report giving a detailed description of the status of the project that is the subject of this Contract. Contractor shall include, at a minimum, in the quarterly progress report: 2.2.1. A short narrative of the original project (this would remain the same for each report submitted); 2.2.2. Project progress during the reporting period (activities carried out, accomplishments, highlights, problems, corrective actions, etc., must be discussed); 2.3. Within 45 days after the project is completed, Contractor shall submit a final report to the State that must include, at a minimum: 2.3.1. A summary of activities performed; 2.3.2. The overall project accomplishments; 2.3.3. Successes and failures, including significant problems or unique situations encountered, and corrective actions taken; and 2.3.4. Extent to which the project goals and objectives were met. 2.4. Contractor shall provide the State with an itemized billing statement when it submits quarterly progress or final reports required by Section 4 (CONSIDERATION/PAYMENT).

SERVICES AND/OR SUPPLIES. Contractor shall provide State the following on-site primary and preventative medical evaluation/management services to youth and adult offenders housed at Pine Hills Correctional Facility (PHCF), one (1) day per week, for not less than one (1) hour per day. In the event a licensed physician is not available, Contractor may provide a Nurse Practitioner to conduct the visits. The specific day of service will be scheduled with the PHCF Nurse Supervisor. Should Physician be unable to make the scheduled on-site visit, Contractor will inform PHCF and reschedule the visit within the same service week. If PHCF requires an additional visit during the week, PHCF staff will schedule the visit at Contractor’s Miles City Health Clinic, informing the Clinic of the number of youth or adult offenders to be seen and the nature of their medical needs. The services provided by Contractor shall also include administrative off-site work such as dictation, record keeping, transcription, phone calls and the like.
