Types of Services. This Article governs the provision of internetwork facilities (i.e., physical interconnection services and facilities), meet point billing by GTE to Nextel or by Nextel to GTE and the transport and termination and billing of Local, IntraLATA Toll, optional EAS traffic and jointly provided Interexchange Carrier Access between GTE and Nextel. The services and facilities described in this Article IV shall be referred to as the "Services."

The services and facilities described in this Article shall be referred to in this Article IV as the "Services."
Types of Services. The Shareholder Servicing Agent shall be ----------------- responsible for performing shareholder administrative and liaison services, which shall include, without limitation:
(a) answering Customer inquiries regarding account status and history, and the manner in which purchases, exchanges and redemptions of Shares may be effected;
(b) assisting Customers in designating and changing dividend options, account designations and addresses;
(c) providing necessary personnel and facilities to establish and maintain Customer accounts and records;
(d) assisting in aggregating and transmitting purchase, redemption and exchange transactions;
(e) arranging for the wiring of money;
(f) transferring money in connection with Customer orders to purchase or redeem shares;
(g) verifying and guaranteeing Customer signatures in connection with redemption and exchange orders and transfers and changes in Customer accounts with a bank which is designated in the Fund Account Application and which is approved by a Fund's Transfer Agent;
(h) furnishing (either separately or on an integrated basis with other reports sent to a Customer by the Shareholder Servicing Agent) monthly and year-end statements and confirmations of purchases, redemptions and exchanges;
(i) furnishing, on behalf of the Shares of a Fund, proxy statements, annual reports, updated prospectuses and other communications to Customers;
(j) receiving, tabulating and sending to a Fund, proxies executed by Customers; and
(k) providing such other related services, and necessary personnel and facilities to provide all of the shareholder services contemplated hereby, in each case, as the Company or the Trust, as the case may be, or a Customer may reasonably request.

The services and facilities described in this Article shall be referred to in this Article V as the "Services."
1.1.1 NEN initiates orders for trunk-side interconnection services by sending an ASR to GTE. NEN should submit ASRs to GTE through on-line applications or electronic files. The ordering process is described in the GTE Guide. The ASR will be reviewed by GTE for validation and correction of errors. Errors will be referred back to NEN. NEN then will correct any errors that GTE has identified and resubmit the request to GTE electronically through a supplemental ASR.

Types of Services. The types of EFT services we provide are listed below. Some of these services may not be available for certain types of Accounts. Please also see the Transfer Limitations subsection in the Consumer Deposit Accounts section of this Account Agreement for other limitations that may apply to these services.
Types of Services. Subject to Licensee paying the applicable fee for Support hereunder for a particular Component System, Infor shall (a) provide Licensee with access (via the Internet, telephone or other means established by Infor) to Infor’s support helpline, (b) provide, when and if generally available, updates, enhancements or modifications to the then-current, general release version of such Component System that are not separately priced or licensed as new products; and (c) use reasonable efforts to correct or circumvent Documented Defects (the foregoing referred to collectively as “Support”).

The following types of health care services will be considered for funding: Sick medical visit (provided by a licensed health care provider) Laboratory and X-ray services Prescription drugs (generic when available) Outpatient medical services (provided by a licensed health care provider) Up to $100,000 may be allocated in the FY’20 service agreement cycle (March 1, 2020 – February 28, 2021). The final amount allocated by the Health Care Foundation will be determined by the Collin County Health Care Foundation Advisory Board and/or Trustees. An applicant may request up to $50,000 in funding. Due to the expected volume of proposals, not all proposals may be funded by the Collin County Health Care Foundation. An unsuccessful application does not reflect the worthiness of a particular project. If you plan to participate in the Medicaid waiver 115 program, you will not be eligible to apply for this grant Annual fund drives or fund raising events; Religious, political or direct lobbying purposes; Rent or lease payments; Grants to individuals; Office equipment e.g. computers, faxes, copiers, etc. Operating deficits; Indirect costs; Renovations; Items/services for which third-party reimbursement is available (e.g. SCHIP, Medicaid, Medicare or private insurance); Attorney and legal fees; Medical or academic research; Mental health services; Vision services; Dental services; Medical supplies and equipment; Case management services; Services to those individuals who are not U.S. Citizens and Resident Aliens with less than 40 working quarters of U.S. residency; Services to those individuals whose family income exceeds 100% of the Federal Poverty Level; and Services to those individuals that are not domiciled in Collin County, Texas. The Collin County Health Care Advisory Board, appointed by the Collin County Health Care Foundation Board of Trustees, will review and rank applications submitted prior to the announced funding selection deadline. The applications with the highest rankings will be forwarded to the Collin County Health Care Foundation Board of Trustees to make the final determination and appropriation of funding. Upon the formal approval of funding by the trustees, a service agreement will be executed between the successful applicant and the Collin County Health Care Foundation. Reimbursement for services will be made on an after-the-fact basis and on a fee-for-service basis.

Pathnet then will correct any errors that GTE has identified and resubmit the request to GTE electronically through a supplemental ASR.
