Common use of Urgent Care Centers Clause in Contracts

Urgent Care Centers. This plan covers care you get in an urgent care center. Urgent care centers have extended hours and are open to the public. You can go to an urgent care center for an illness or injury that needs treatment right away. Examples are minor sprains, cuts and ear, nose and throat infections. Covered Services include the doctor’s services. You may have to pay a separate copay or coinsurance for other services you get during a visit. This includes things such as x-rays, lab work, therapeutic injections and office surgeries. See those covered services for details. If an urgent care visit is provided in a center located in a hospital, benefits may also be subject to the calendar year deductible and coinsurance related to facility fees charged by the hospital. This plan covers services you get in a hospital. At an in-network hospital, you may get services from doctors or other providers who are not in your network. When you get covered services from non- contracted providers, you pay any amounts over the allowed amount. Some services require prior authorization. See
