4.2.1.1.2.1. DPHE will coordinate with HCPF on the development of informational materials affecting Medicaid populations.

4.2.1.1.3.1. HCPF shall provide DPHE with the program data necessary to comply with all federal and state reporting requirements necessary to administer the program.
4.2.1.1.3.2. HCPF shall provide DPHE a quarterly list of Medicaid enrolled providers by the fifteenth (15) Business Day of the last month in each state designated fiscal quarter.
4.2.1.1.3.2.1. Provider data shall include provider name, clinic/facility name, location address, telephone and fax number.
4.2.1.1.3.2.2. HCPF shall co-manage the joint federal technical assistance grant known as Community of Practice (CoP) to increase immunization rates among children and pregnant women enrolled in Medicaid.
4.2.1.1.3.2.2.1. As part of the CoP grant, HCPF shall provide three files (Patients, Services and Providers) to DPHE as requested not more than twice annually). These files are nearly identical to the files shared with DPHE on a weekly basis in sections “4.2.1.1.3.1.”, 4.2.1.

If Provider is a primary care provider who serves Covered Persons under age nineteen (19), Provider shall administer vaccines consistent with the American Academy of Pediatrics (AAP)/Bright Future periodicity schedule. Provider is additionally encouraged to participate in the Vaccines for Children (VFC) program, which allows providers to receive vaccines at no cost for children eligible for Medicaid who are under age nineteen (19). Vaccines for Children. (2 through 18 years of age). Vaccines may be offered based on the ACIP schedule and guidelines as well as scope of work, eligible population and as approved by the Client.

Vaccines for Children. Through the VFC program, the Federal and State governments purchase, and make available at no cost, vaccines for AHCCCS children under age 19. A provider may impose a charge for the administration of a qualified pediatric vaccine as stated in 1928(c)(20)(C)(ii) of the Act. Contractors are required to adopt the payment rates in the Arizona Medicaid State plan, as described on Page 66b, as a minimum fee schedule for VFC providers.
Vaccines for Children. The HEALTH PLAN agrees to participate in the Vaccines for Children (VFC) immunization program, pursuant to 42 U.S.C., Section 1396s. The HEALTH PLAN will collaborate as reasonably requested with public health agencies to ensure childhood immunizations to all enrolled families with Children, pursuant to Minnesota Statutes, Section 256L.12, Subdivision 10.
