In consideration for receiving permission to release my Kindergarten age student (name) ____________________________________ (herein referred to as STUDENT), at their bus stop without authorized parent/guardian or alternate person being present I, hereby release, waive, discharge, covenant not to sue, and agree to hold harmless for any and all purposes the Centralia/Chehalis Pupil Transportation Co-operative, represented School Districts, the Board of Directors from both the Centralia and Chehalis School Districts, and their officers, agents, volunteers or employees (herein referred to as RELEASES), from any and all liabilities, claims, demands, or injury, including death, that may be sustained by the named student.
