Thank you for your interest in participating as a Supervisor in American Denturist College’s (“ADC”) externship program (the “Externship Program”). Enclosed please find ADC’s Supervisor Application, Agreement and Release. Please read the Agreement carefully. If you agree to its terms, please complete the Agreement and return it to ADC. You must submit the completed Agreement to ADC prior to beginning direct supervision and training of any extern. Please let us know if you have any questions regarding the Agreement.
