The undersigned parents’ (“The undersigned”) son or daughter, , (“The Enrollee”) is enrolled as a student or participant at an educational, athletic or outdoor program (Program Name: ) (“The Program”) offered by or through Memphis Thunder Aquatic Club. The undersigned acknowledge that The Program’s on and off campus functions are integral to the success of The Program and the furtherance of The Enrollee’s educational, athletic or outdoor goals and therefore consent and agree to The Enrollee’s participation in The Program subject to the terms of this agreement.
