I, , hereby grant Nationwide Retirement Solutions, Inc. (NRS) and the Kentucky Public Employees’ Deferred Compensation Authority (KDC) including their agents, licensees, affiliates, successors and assigns (collectively “Assignees”) the irrevocable and unrestricted right to use and publish the following testimonial statement, photographs, or excerpts, including my image or likeness , in all media broadcasted, published, or distributed nationally in print, electronically, and in all new media utilized and in the advertising and promotion as required by Assignees. I hereby acknowledge that I have the right to consent and participate in this testimonial, to disclose the information provided, and to grant any permission requested. I hereby release Assignees from all claims, fees, and liabilities associated with the use of this testimonial.
