THIS BUSINESS ASSOCIATE AGREEMENT (“Agreement”) is entered into on the date the Agreement is fully executed between Texas Tech University Health Sciences Center El Paso (“TTUHSC El Paso” or “Covered Entity”) and Company Name (“Business Associate”). Both TTUHSC El Paso and Business Associate are also referred to herein as “Party” or, collectively, “Parties.”
