For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I give NHL Enterprises, L.P., on behalf of itself, the National Hockey League, its member clubs, and their respective licensees, assigns, successors, partners, sponsors, affiliates, agencies, agents and designees (collectively, “NHL”), the irrevocable permission, authorization and/or license to film, video, photograph, make use of and/or otherwise record me, my voice, my likeness, my performance and any other biographical or personal information or materials (the “Materials”) during and in connection with my appearance in the shoot/filming for content to be used in connection with NHL games, events and promotional content taking place on through NHL virtually recording my performance in my place of residence (the “Program”) as follows:
