This Accountholder Agreement (this “Agreement”) and the above Schedule of Fees constitute our disclosure to you and an agreement between you and us under which you establish one or more electronic deposit accounts (called an “Account” at Republic Bank of Chicago through Retail Locations or the internet. Please read this Agreement carefully and keep it for future reference. By applying for, maintaining, and using this Account, you represent and warrant to us that: (i) you are citizen or permanent resident of the fifty (50) United States or the District of Columbia who can lawfully enter into and form contracts under applicable law in the state in which you reside; (ii) the personal information that you have provided to us is true, correct and complete;
