This master purchasing agreement (“Agreement”) to furnish certain goods and services described herein and in the documents referenced herein (“Goods and/or Services”) is made by and between UC Health, an operating subdivision of the Regents of the University of California, a California public corporation (“UC”), with a location of 1111 Broadway, 14th Floor, Oakland, California 94607, acting for the benefit of UC locations and UC affiliate locations as defined herein and the supplier, [NAME OF SUPPLIER], a ______ corporation, located at [ADDRESS OF SUPPLIER] (“Supplier”). UC and Supplier shall individually be referred to as “Party” and collectively, “Parties.”
