In consideration of my child’s participation in the Ski and Snowboarding Club, I hereby, for my son/daughter, their heirs, executors, and administrators, waive and release any and all rights and claims for liability and damages I may have against the OREGON CITY SCHOOL DISTRICT #62; its employees; agents; officers; sponsors; volunteers; Oregon City School district #62, including Gardiner Middle School, middle school campus; and assigns for any and all injuries or death suffered by my son/daughter in or arising from said activity and/or event. I acknowledge that it is my responsibility to understand the risks and determine whether my child is fit to safely participate in this activity and any ensuing events. I attest and certify that my son/daughter is physically able to safely participate in this activity and any ensuing events and has properly prepared to undertake this activity and any ensuing events.
