THIS LICENSE AGREEMENT (“Agreement”), effective as of the effective date listed on Schedule A (“Effective Date”), is entered into by and between Integrity Health Strategies, LLC (“IHS”), and “Licensee,” identified in the attached Schedule A (each a “Party” and collectively the “Parties”).
