By signing below, I take responsibility for all people listed on this membership. This responsibility includes key cards, and any damage to village property due to negligence on the part of any person covered by this membership. I also acknowledge that no one under the age of 18 is allowed in the fitness center without a parent or guardian and that only the people listed on this membership form are allowed to access the fitness center. There are no guests allowed.
