Each license is for a calendar year (January 1 through December 31) to individual, licensed instructors—not to locations. Rates are not prorated and renewals are due by the end of January. Each licensed instructor takes the license with him or her from location to location and job to job. There is a slight increase from year to year. The current year form must be used.
