1, , am the parent or lawful guardian of the above-named participant ("my child"), and I hereby grant permission for my child to participate in the Physical Education class of Veritas Christi, held at the People of Praise building in Rockford, IL, and at other suitable venues. If my child is enrolled in the “Projects Class” as a student at Veritas Christi or is a participant in Action, I grant permission for them to participate in its activities, which may include service, building skills, and light construction, including the possible use of power tools and ladders, always with adult supervision and a thorough safety analysis. There may be additional activities off of the campus of the Veritas Christi Hybrid Academy that may also involve physical activity. I also grant permission, that if my child needs to be transported to a location other than the People of Praise building at which such activities occur, I hereby grant my permission for them to be transported there by an adult, 18 o
