This form is to be completed for anyone under the age of 19 as of August 31, 2020, must complete the following waiver prior to participating in any events at the Cloverdale Curling Club during the 2020/21 season. A legal guardian must complete the form on behalf of the minor. The Cloverdale Curling Club (hereinafter referred to as “the Organization”).
