I, (please print), hereby agree to adopt the animal(s) referenced below (hereafter “animal”) from FOR ANIMALS INC and release and indemnify FOR ANIMALS INC and its officers, directors, trustees, employees, volunteers and agents from and against all claims for property damages and/or for personal injury to myself or others associated with this adoption or who have access to the animal. While under my care, the welfare of this animal will be my sole responsibility. I am not adopting this animal for another person, nor will this animal be used for breeding, medical or experimental research or any other purpose. I understand that in the event I give false information or do not follow the terms of this contract, FOR ANIMALS INC may reclaim this animal and begin legal proceedings against me at my expense and may assess damages up to the maximum permitted by law resulting from any false information or breach of the terms of this contract.
