This is a request to bring a non‐JSerra student to a JSerra dance. All guests must be 20 years or younger. Complete the request fully, have it signed by an administrator from the guest’s high school (if applicable), attach a photo id, and return the request to the Dean no later than the October 12, 2016. LATE CONTRACTS WILL NOT BE ACCEPTED.
