We write in relation to the provision of the MyDiabetes MyWay platform (the “Platform”), supplied by MyWay Digital Health (“we”, “us”, “our”, “MWDH”), paid for by the Funding Organisation (“You”), to Patients within the boundary of the “Area” considered as the area covered by GP Practices within the organisation(s) listed in the table below. A list of nominated individuals who are responsible for the management and governance of certain aspects of this contract is in Appendix 1.
