This Amendment to the above-referenced Original Agreement (hereinafter called the “Agreement”) is entered into by and between the Department of Health Care Policy and Financing, 1570 Grant Street, Denver, CO 80203 (hereinafter called “HCPF” or the “Department”), and the Archuleta County (hereinafter called “Contractor”), who may collectively be called the “Parties” and individually a “Party”, both of which are agencies of the STATE OF COLORADO, hereinafter called the “State”.
