In consideration of being allowed to participate in Activities at River Valley Ranch for the date(s) of I, for myself and any minor children for whom I am parent, legal guardian, or otherwise responsible and for my/our heirs, personal representatives or assigns, hereby acknowledge the risks of injury or damage (to property or personal injury) involved in participating in the paintball and/ or Archery Tag activities offered by River Valley Ranch. I further understand that participation in these activities contains risks we each appreciate and voluntarily assume because the minor and we choose to do so. My participation in such activities and/ or use of such equipment may result in my injury or illness including but not limited to bodily injury, disease strains, fractures, partial and/or total paralysis, eye injury, blindness, heat stroke, heart attack, death, or other ailments that could cause serious disability.
