I, the undersigned, hereby agree to play for the team know as: in the TCPUD Department of Parks and Recreation Athletic League. In contracting to play for the above team, I agree not to contract with any other team, unless released from the above, in accordance with the league rules. I release the TCPUD Department of Parks and Recreation officials, players and spectators from any and all liability for accident or injury that I may suffer as a result of my participating in this league. I agree to abide by the TCPUD Department of Parks and Recreation, "Adult Code of Conduct" and have read and agree to abide by all the league rules and department policies. I also attest that the following information is true to the best of my knowledge.
