This agreement shall be for the following period (not to exceed twelve months) and may be renewed in one year periods or shorter at the discretion of the supervisor if requested by the employee. All flextime agreements shall begin on the first day of the month and end on the last day of a month. This agreement should be reviewed and revised as needed if there is a change in supervisor, job responsibilities, or change in work circumstances or performance.
