This Commitment Form (“Commitment Form”) is made by the undersigned entity, in its capacity as an Eligible Member, for the benefit of AstraZeneca Pharmaceuticals LP (“AstraZeneca”), a Delaware limited partnership, as of the Effective Date. This Commitment Form sets forth certain terms and conditions governing the Committed Pricing Options offered by AstraZeneca for Eligible Members under the current Agreement between Minnesota Multistate Contracting Alliance for Pharmacy (“GPO”) and AstraZeneca (the “Agreement”) and the ZOLADEX Product Schedule thereto (the “Product Schedule”) with respect to ZOLADEX. All capitalized terms not defined in this Commitment Form shall have the meaning set forth in the Product Schedule and/or the Agreement.
