ABC Benefits Corporation (“Alberta Blue Cross”) makes the Alberta Blue Cross Physiotherapy Online Services Web Site (the “Site”) available solely for the purpose of providing physiotherapist practitioners the means to obtain pre-determinations and/or claim adjudication on behalf of patients covered under an Alberta Blue Cross physiotherapy benefit plan. As a condition of using the Site, the following terms and conditions of this Physiotherapy Online Services Billing Agreement (the “Agreement”) must be adhered to. Your use of this Site signifies that you have read, understood and agree to be bound by the terms of this Agreement.
