The undersigned (name of participant) desires to participate in various programs, events or activities (hereinafter collectively referred to as "activities) operated by Faith Mission Int. or sponsored by (Participant's church, or organization name) (hereinafter referred to as the "Mission Church"). The undersigned further understands and acknowledges that the undersigned (and/or children and/or minors) may incur personal injury or bodily damage while participating in such "activities." The undersigned further understands and acknowledges that the "Mission Church" will not allow the undersigned (and/or children and/or minors) to participate in such "activities" without releasing and holding harmless the "Mission Church". Further, the undersigned requests that the "Mission Church activities" in consideration thereof agree to hereby release, and forever discharge the "Mission Church" their officers, directors, any employees, their agents, and any parties volunteering on behalf of the "Mis
