The State of Hawaii Department of Health, (COVERED ENTITY) has determined it is a Covered Entity or Health Care Component of a Covered Entity under the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191 (HIPAA), as amended, and its implementing regulations at 45 CFR Parts 160 and 164 (the HIPAA Rules).
