This agreement is made effective as of the date or dates identified in the Show Dog Synergy Dog Authorization form attached by reference to this agreement. This agreement is by and between Show Dog Synergy LLC (the “Provider”) of P. O. Box 211111, Bedford, Texas 76095, and the “Client” as identified in the attached applicable Show Dog Synergy Dog Authorization form which makes up a part of this agreement / contract.
