RISK SHARING AGREEMENT ON PRESCRIPTION DRUGS: 5 YEARS EXPERIENCE OF THE KOREA NATIONAL HEALTH INSURANCE

Objectives: In 2014, the Korea National Health Insurance Service (NHIS) introduced the RSA (Risk Sharing Agreement) to ensure patient access to new drugs while holding the pharmaceutical industry responsible for the part of financial burden that could arise from uncertain performance of the drug. Only the prescription drugs indicated for cancer and rare diseases were eligible for the RSA. We aimed to document the outcomes of the RSA to inform the stakeholders of potential changes for cost-effective implementation of RSA. Methods: All the RSAs that the NHIS has finalized until June 1, 2019 were included. They were classified in terms of agreement type, indication, drug class (ATC 5 category), and the origin of the agreement holder. The agreement type was budget cap, fixed amount refund per unit sold and CED (Coverage with Evidence Development). The origin of the agreement holder was either domestic or global. Abstract: Objective: The National Cardiovascular Data Registry indicates bleeding complications occur among 5.8% of patients undergoing percutaneous coronary intervention (2008–2011); however, trials have demonstrated that the VASCADE® Vascular Closure System (VCS) lowers rates of access site closure–related complications. With an estimat- ed cost of $5,440 per bleeding complication, reducing complications may lead to substantial cost savings. By connecting quality metrics to costs, risk-sharing agreements (RSAs) can improve outcomes and patient satisfaction while managing financial risk. Here, we describe real-world experience and practical implications of femoral arterial vascular closure using the VCS by examining qualifying complications from participating institutions enrolled in a novel RSA, the Performance Guarantee (PG) partnership, reported during 2016–2020.

We study the possible rise of cooperation in risk-sharing agreements where, in the first stage, the ”pool™ collectively chooses the level of reimbursement and, in the second stage, each member individually selects a costly loss-mitigating action which is not contractible. The action can be chosen either non-cooperatively or cooperatively and the game is repeated an indefinite number of periods. We first show that the pool offers partial coverage under non-cooperation and full coverage under cooperation. Then we show that, in the repeated game, cooperative behaviors improve efficiency by reducing the total amount to be reimbursed by the pool in each period but they also generate strong free-riding incentives that are increasing in the pool size. We thus identify a new interesting trade-off: welfare generated by the risk-sharing agreement is increasing with the size of the pool, but at the same time the pool size must not be too large for cooperation to be self-enforcing. SUMMARY. Risk Sharing Agreement (RSA) is an increasingly used method of reaching an agreement between payers and marketing authorization holders (MAH). RSAs are generally confidential, because of external price referencing, making it difficult to access scientific publications from which information about their structure, details, and mechanisms could be obtained. By introducing the RSA, payers aim to provide patient access, but at a confidential and fair price. RSA can be divided into financial-based RSA and performance-based RSA. Each of the different types of agreements has its advantages and disadvantages.
