I, the person signing this Adoption Agreement (hereinafter called the “Participant”), establish an Individual Retirement Account (the “Account”) with UMB Bank, n.a., as Custodian (“Custodian”), to operate in conjunction with a SIMPLE IRA plan established by my employer or to receive a transfer from another SIMPLE IRA. I agree to the terms of my account, which are contained in the document entitled “UMB Bank, n.a., SIMPLE Individual Retirement Custodial Account” and this Adoption Agreement. My account will be effective upon acceptance by the custodian.
