It is preferable that faculty provide testing accommodations. However, this is not always possible due to scheduling conflicts and other various issues. The purpose of the OSSD Proctoring Center is to support faculty with providing testing accommodations for students with disabilities.

This authorization form is only for test sites that are not on the pre-approved remote testing sites list. This form must be completed in its entirety and uploaded into the TSI Registration form for Option #2 of Remote Testing. If this form is not completed correctly, it will delay setting up your test with the proctoring institution.

The Office of Services to Students with Disabilities (SSWD) requires each student to have a current term accommodation letter on file before the SSWD Testing Center will provide any exam accommodations. The SSWD Testing Center is located in the lower level of the Library in Room 14. The entrance of the Testing Center is located on the south side of the Library across from the AIRC Building.
