Acknowledgement of Risk. I, meaning an adult participant, understand and acknowledge that participating in the (herein “ACTIVITY”), during the term of this agreement, at sponsored by Xxxxx-Xxxxx State College (herein “LC State”) entails both known and unanticipated risks. Acknowledgement of Risk. I understand that the Activity involves certain risks, hazards and conditions that may be dangerous to life, limb and property and that can arise in an incalculable variety of unforeseen or foreseeable ways, which may include: bodily injury, loss of limb, death or property damage. These risks specifically include, but are not limited to, the following: Cuts and Bruises, Broken Bones, Torn Ligaments, Muscle Injuries, Visual Impairments, or any other unforeseeable injury. I am participating in the Activity with knowledge of the dangers involved. I have reached the age of majority, and I am competent to make this decision for myself. Medical Release: I am not suffering from any medical condition, impairment, or disease that would prevent my safe participation in the Activity. Acknowledgement of Risk. We understand that we are choosing to participate in athletics voluntarily and while necessary pre-cautions are adhered to at every athletic event, there is always a risk of injury traveling to/from and during athletic competition.
