Cancellation Form means the form which we supply as part of your Estate Pack and which shall be used by you to end your Plan;

• A Resident who cancels this Contract prior to the end of the Contract Term agrees to pay any applicable cancellation fees and submit a Contract Cancellation Form with the UHRL Office prior to check out.
