Continuing treatments definition

Continuing treatments include: (a) two or more visits to a health care provider;(b) two or more treatments by a health care practitioner (e.g., physical therapist) on referral from, or under the direction of a health care provider; or
Continuing treatments means: 19 1) Two or more visits to a health care provider; 20 2) Two or more treatments by a health care practitioner (e.g., 21 physical therapist) on referral from or under the direction of a health care provider; or 22 3) A single visit to a health care provider that results in a 23 regimen of continuing treatment under the supervision of the health care provider.

EligibilityTo be eligible for family and medical leave, an employee must have been employed by the City of Concord for at least 12 months, for at least 1250 hours of service during the 12 month period immediately preceding commencement of family and medical leave.
