De-identified health information definition
De-identified health information means health information that does not identify an individual and with respect to which there is no reasonable basis to believe that the information can be used to identify an individual.
De-identified health information means information that does not identify an individual provider, patient, member or enrollee. De-identification means that such health information shall not be individually identifiable and shall require the removal of direct personal identifiers associated with patients, members or enrollees.
