Proof of age The Company may require evidence of Age of any Annuitant or Owner.
Proof of Claim means a proof of Claim Filed against any of the Debtors in the Chapter 11 Cases.
Proof of Disability means documented clinical findings that prove that You are Disabled.
Proof of Concept has the meaning set forth in Section 2.3.
Proof of vaccination means documentation provided by a tribal, federal, state or local government, or a health care provider, that includes an individual’s name, date of birth, type of COVID-19 vaccination given, date or dates given, depending on whether it is a one-dose or two-dose vaccine, and the name/location of the health care provider or site where the vaccine was administered. Documentation may include but is not limited to a COVID-19 vaccination record card or a copy or digital picture of the vaccination record card, or a print-out from the Oregon Health Authority’s immunization registry.
proof of B-BBEE status level of contributor means:B-BBEE Status level certificate issued by an authorized body or person;A sworn affidavit as prescribed by the B-BBEE Codes of Good Practice;Any other requirement prescribed in terms of the B-BBEE Act;
Annual Information Form means a document filed by the funds with Canadian securities regulators which provides supplementary information about the funds.
Retirement Eligibility means attainment of age 60 and completion of at least five (5) years of continuous service with the Company.
Proof of payment means, as applicable, a copy of the check, confirmation of credit card or debit card payment, or confirmation of wire or automated clearinghouse transfer, and any other information required to demonstrate that payment has been made according to EPA requirements, in the amount due, and identified with the appropriate docket number and Respondent’s name.
Year of Eligibility Service means, for any Special Employee, a credit used to determine his eligibility to participate under the Plan, as further described in Section 2.2.
Social Security Benefit means an amount received by a claimant as a monthly benefit in accordance with the Social Security Act, 42 U.S.C. Sec. 401 et seq.
Retirement Eligible means that the Participant has either attained age 55 and completed ten (10) years of Service as an Employee or attained age 60 and completed five (5) years of Service as an Employee.
Social Security Benefits means any social insurance, pension insurance benefits, medical insurance benefits, work-related injury insurance benefits, maternity insurance benefits, unemployment insurance benefits and public housing reserve fund benefits or similar benefits, in each case as required by any applicable Law or contractual arrangements.

Registration Form means a book voter registration form and a by-mail voter
Proof of Interest means a proof of Interest Filed against any of the Debtors in the Chapter 11 Cases.
Statement of Additional Information means, respectively, the form of prospectus and statement of additional information with respect to the Fund filed by the Investment Company as part of the Registration Statement, or as they may be amended or supplemented from time to time.
Account Application Form means the Personal Account Application Form for International Personal Banking or Wealth and Investment as applicable;
Proof of Loss means satisfactory written proof that a loss occurred for which the Group Policy provides benefits, which is not subject to any exclusion, and which meets all other conditions for benefits. Proof of Loss includes any other information We may reasonably require in support of a claim for benefits under the Group Policy.
Primary Social Security Benefit means, with respect to any member, the primary insurance amount
Medicare eligible expenses means expenses of the kinds covered by Medicare Parts A and B, to the extent recognized as reasonable and medically necessary by Medicare.
Notice-triggering Personal Information means the personal information identified in Civil Code section 1798.29 whose unauthorized access may trigger notification requirements under Civil Code section 1798.29. For purposes of this provision, identity shall include, but not be limited to, name, address, email address, identifying number, symbol, or other identifying particular assigned to the individual, such as a finger or voice print, a photograph or a biometric identifier. Notice-triggering Personal Information includes PI in electronic, paper or any other medium.
Evidence of Eligibility means evidence of an individual's eligibility for training subsidised through the Skills First Program in accordance with the eligibility requirements set out in this VET Funding Contract and the Guidelines about Determining Student Eligibility and Supporting Evidence.
