  • Qualifying Shareholders who are not Eligible CSN Shareholders, and Eligible CSN Shareholders who submit a valid Form of Election to opt out of the Jackson Nominee Service, will receive a direct registration statement in respect of their Jackson Shares.

  • If you wish to receive your Jackson Shares, held in the form of Jackson CDIs within the Jackson Nominee Service, you do not need to complete this Form of Election.

  • Community Development Director means the Director of City’s Department of Community Development or his or her designee.
