Notice to Insurers definition

Notice to Insurers means a notice letter on the Provider's corporate letterhead in substantially the form attached hereto as Exhibit VII-B.
Notice to Insurers means a notice letter on each Health Care Provider's ------------------ corporate letterhead in substantially the form attached hereto as Exhibit I or --------- in such other form combined with any notice to Insurer in respect of payment instructions containing language substantially similar to that contained in such form.

  • Certificate of Insurance means a document showing that an insurance policy has been written and includes a statement of the coverage of the policy.
