Service Report Form definition

Service Report Form or “SRF” means the form which is completed and duly acknowledged by the End-User upon completion of the installation of the Termination Point;

Examples of Service Report Form in a sentence

  • Personnel performing the pest control service shall report to the Facility Contract Person each time service is performed and shall provide a Service Report Form showing areas services, infestation if any found, and corrective action taken.

  • Upon completion of remedial work a duly authorised representative of the Customer shall, if the Supplier so requests, sign a Service Report Form (in such form as may be specified by the Supplier) in duplicate confirming the Customer’s instructions to the Supplier to carry out such work and specifying the work completed by the Supplier to the Customers satisfaction.

  • Benchmarking Report means the report produced by the Supplier following the Benchmark Review and as further described in this Framework Schedule 7;

  • Electronic Subcontracting Reporting System (eSRS) means the Governmentwide, electronic, web-based system for small business subcontracting program reporting. The eSRS is located at http://www.esrs.gov.

  • MI Report means a report containing Management Information submitted to the Authority in accordance with Framework Schedule 8 (Management Information);

  • Root Cause Analysis Report means a report addressing a problem or non-conformance, in order to get to the ‘root cause’ of the problem, which thereby assists in correcting or eliminating the cause, and prevent the problem from recurring.

  • MI Reporting Template means the form of report set out in the Annex to Framework Schedule 8 (Management Information) setting out the information the Supplier is required to supply to the Authority;

  • Development Report means a written account of Licensee’s progress under the Development Plan having at least the information specified on Appendix B to this Agreement, and shall be sent to the address specified on Appendix B.

  • GLJ Report means the independent engineering reserves evaluation of certain oil, NGL and natural gas interests of the Company prepared by GLJ dated February 12, 2021 and effective December 31, 2020.

  • Pick Up Report means the report detailing the number of sleeping rooms per day actually used out of the Program’s room block.

  • Summary Subcontract Report (SSR) Coordinator, as used in this clause, means the individual at the department or agency level who is registered in eSRS and is responsible for acknowledging or rejecting SSRs in eSRS for the department or agency.

  • Project Management Report means each report prepared in accordance with Section 4.02 of this Agreement;

  • Progress Report means the report similar in form and content to that attached hereto as Appendix III.

  • Catalog Pricing and Pricing Requirements This is a requirement of the TIPS Contract and is non-negotiable. In this solicitation and resulting contract, "Catalog Pricing" shall be defined as: "The then available list of goods or services, in the most current listing regardless of date, that takes the form of a catalog, price list, price schedule, shelf-price or other viewable format that:

  • Rapidly report means within 72 hours of discovery of any cyber incident.

  • Project Report means a summary statement of the likely environmental effects of a proposed development referred to in section 58;

  • Cost Report means a report detailing all costs incurred in relation to the Activity, to be submitted by the Incubatee to the Incubator.

  • Status Report has the meaning set forth in Clause 4.1.
