Settlement Claim Form definition

Settlement Claim Form means the document, in substantially the same form as Exhibit 9 attached to this Agreement, to be completed by a Class Member in order to make a Settlement Claim pursuant to this Settlement.
Settlement Claim Form means an electronic or paper document, in the form approved by the Court, to be completed by Settlement Class Members and submitted to the Claims Administrator to receive a share of the settlement. The Parties agree to propose approval of the Settlement Claim Form attached as Exhibit 5.
