Arriving Late. If you are going to be more than 10 minutes late, please call me to let me know, or I may not be available when you arrive. Fees I charge $200 for a 50-minute session. I prefer that you pay at the time of the session. If you would like to set up a different arrangement, please let me know. Once we schedule an appointment, you will be expected to pay for it unless you provide 48 hours advance notice of cancellation. I charge my regular fee for other professional services, including phone calls lasting longer than 10 minutes, report writing, attendance at meetings with other professionals you have authorized, etc.
Arriving Late. Arriving late to lessons will be recorded by the subject teacher in the class book. A late arrival must also be excused in writing, as described under (2). If this is not the case, the absent days will be assessed as the pupils being unexcused.
Arriving Late. If your group will be late to arrive, please call the The Pub and ask for the Manager on Duty as soon as possible. Your food preparation timeline is based on your scheduled arrival. We cannot guarantee the quality of the prepared food if your party arrives late. Likewise, there may be additional delays in food service. Access and Decorations By permission only, we do allow our guests into the banquet room prior to a scheduled event. We do not allow confetti, flower petals, rice, silly string, tape, nails, or anything affixed to the walls, floors, ceilings, etc. No open flame; candles are permitted within table centerpieces only and must be contained in glass. Any damages to the premises or equipment owned by The Pub & Restaurant will be charged to the host of the event. Charges for additional cleanup or damages will be assessed and billed up to 14 days following the event. The Pub is happy to make arrangements for custom decor, floral design, rentals, etc on your behalf. Please contact the Banquet Manager for more information. These items will be added to the final invoice along with a surcharge. If you should decide to cancel your event and have contracted special services through The Pub, you are still responsible for these fees if said services cannot be cancelled.

Failing to Show Up for Your Appointment If you fail to show up for your appointment without calling or canceling online through the MyHealthPoint patient portal at least 24 hours in advance, you will not be eligible for another scheduled appointment, but you may be eligible to be seen on a space- available “work-in” basis. After keeping your “work-in” appointment, you will be able to schedule other dental appointments in advance, subject to availability of appointment slots. When You Are In The Dental Clinic • Your (or your child’s) first appointment is normally for an examination and evaluation only. • Parents are welcome in the dental exam room during a child’s first visit, but other young children are not normally allowed in the exam room. • Children are not allowed in the exam room while a parent is being treated. • When children are brought to the clinic for treatment, a parent or guardian or other adult authorized to consent to the child’s care shall be present in the clinic facility at all times. Important questions may arise concerning treatment options and the parent or guardian must be available. • HealthPoint is not responsible for unattended children in our reception area. Please arrange to bring another responsible adult or to leave young children at home if you are receiving treatment. I understand and agree to abide by this policy. Arriving Late. Do not reserve a flight that arrives after your team’s scheduled rendezvous time.
