Fully vaccinated. For the purposes of this agreement, “fully vaccinated” means two (2) weeks have passed since the employee received their final dose of either a two (2) dose vaccination (Pfizer-BioNtech, Moderna) or single dose vaccination (Xxxxxxx & Xxxxxxx).

Fully vaccinated. The individual has received the second dose of a COVID-19 vaccine that is a two dose vaccine, or a COVID-19 vaccine that is a single dose vaccine. The vaccine must be a COVID-19 vaccine that is authorized or approved for use by the U.S. Food and Drug Administration. When the above process is satisfied the WMC Director or above will verify the information and then send an email to the Security Department attesting that all of the requirements have been met and the WMC Director is requesting the issuance of a temporary badge to identified specific individuals who have met the requirements. Upon receipt of that email by security, arrangements will be made for the issuance of a Green Colored contractor badge that will self-expire in six (6) months. When it self-expires, request for renewals for the same personnel will not regularly be granted. Rather the process indicated below for longer-term contracted employees will be initiated. For those contracted employees who will be assigned on a regular, longer term or full-time basis to WMC, they must undergo a vetting process conducted by our outside vendor “Green Security.” Information about that process is attached. Once Green Security completes their required process, WMC security may issue a WMC ID after confirming active participation and clearance in the Green Security System.

Fully vaccinated. I understand that if I travel outside of the United States I will stay home and get tested 3-5 days after arrival in the U.S. and must provide Pathways with a negative COVID-19 test result before entering the facilities to drop off / pick up my child. If I have test results to share, I will call the COVID-19 Reporting Line (see below) for further instruction.

  • Influenza Vaccination The parties agree that influenza vaccinations may be beneficial for patients and employees. Upon a recommendation pertaining to a facility or a specifically designated area(s) thereof from the Medical Officer of Health or in compliance with applicable provincial legislation, the following rules will apply:

  • Vaccinations Contractor understands, acknowledges, and agrees that, pursuant to Article II of the General Appropriations Act, none of the General Revenue Funds appropriated to the Department of State Health Services (DSHS) may be used for the purpose of promoting or advertising COVID-19 vaccinations in the 2024-25 biennium. It is also the intent of the legislature that to the extent allowed by federal law, any federal funds allocated to DSHS shall be expended for activities other than promoting or advertising COVID-19 vaccinations. Contractor represents and warrants that it is not ineligible, nor will it be ineligible during the term of this Contract, to receive appropriated funding pursuant to Article II.

  • Hepatitis B Vaccine Where the Hospital identifies high risk areas where employees are exposed to Hepatitis B, the Hospital will provide, at no cost to the employees, a Hepatitis B vaccine.

  • Influenza Vaccine Upon recommendation of the Medical Officer of Health, all employees shall be required, on an annual basis to be vaccinated and or to take antiviral medication for influenza. If the costs of such medication are not covered by some other sources, the Employer will pay the cost for such medication. If the employee fails to take the required medication, she may be placed on an unpaid leave of absence during any influenza outbreak in the home until such time as the employee has been cleared by the public health or the Employer to return to the work environment. The only exception to this would be employees for whom taking the medication will result in the employee being physically ill to the extent that she cannot attend work. Upon written direction from the employee’s physician of such medical condition in consultation with the Employer’s physician, (if requested), the employee will be permitted to access their sick bank, if any, during any outbreak period. If there is a dispute between the physicians, the employee will be placed on unpaid leave. If the employee gets sick as a reaction to the drug and applies for WSIB the Employer will not oppose the application. If an employee is pregnant and her physician believes the pregnancy could be in jeopardy as a result of the influenza inoculation and/or the antiviral medication she shall be eligible for sick leave in circumstances where she is not allowed to attend at work as a result of an outbreak. This clause shall be interpreted in a manner consistent with the Ontario Human Rights Code.

  • Vaccination and Inoculation (a) The Employer agrees to take all reasonable precautions, including in-service seminars, to limit the spread of infectious diseases among employees.

  • TEACHING HOURS AND TEACHING LOAD A. As professionals, teachers are expected to devote to their assignments the time necessary to meet their responsibilities, but they shall not be required to “clock in or clock out” by hours and minutes. Teachers shall indicate their presence for duty by placing their signature and time in the proper column of the faculty “sign-in” roster.
