U79 – DRIVE Version: CV2 The Company agrees to include the following language as a Letter of Agreement (LOA22) with the changes outlined below: The Company agrees to payroll deduct voluntary contributions to D.R.I.V.E. from all employees who submit authorization cards specifically authorizing the deductions and the disclosure of their Social Security Number to D.R.I.V.E. with reports of the deductions. D.R.I.V.E. will notify the Company of the amounts that are to be deducted, as designated by each contributing employee. The Company shall subsequently transfer the funds, along with each employee’s name, Social Security Number, and total amount deducted on each employee’s behalf, on a monthly basis to: National D.R.I.V.E.
