Rehabilitation Plan. If the chief executive officer makes the determinations necessary to provide rehabilitation services under this section and rehabilitation is agreed to by the parties, the retirement system shall designate a rehabilitation provider to evaluate the person and develop a rehabilitation plan. Rehabilitation Plan. The parties agree to adopt the Preferred Schedule of Supplemental Contributions and agree to contribute to the National Industry Plan in accordance with any updated Preferred Schedule of Supplemental Contributions rate table that may be adopted.

Contributions required by this provision will be paid to the Fund on or before the fifteenth (15th) day of the month following the period for which contributions are due or before such other date as the Trustees may hereafter determine. Contributions will be transmitted together with a remittance report containing such information, in such manner and on such form, as may be required by the Fund or their designee.
Rehabilitation Plan. (a) The Care Planning Team is responsible for evaluating if a Client needs a Rehabilitation Plan. Rehabilitation Plans may be needed by Clients who receive therapy provided under direction of licensed therapists and the therapy plan requires regularly scheduled interventions by direct care staff such as but not limited to:
