Claim Forms definition

Claim Forms means the form or forms to be approved by the Court, which when completed and submitted in a timely manner to the Administrator, enable(s) a Class Member to apply for compensation from the Net Settlement Amount.
Claim Forms mean the form Class Members (including Minor Subclass Members) must timely submit to receive payment under this Settlement Agreement. The Claim Form must be substantially similar to the form attached as Exhibit 5.
Claim Forms means Claim Form–A, Claim Form-B, Claim Form-C, and Claim Form-D.

Examples of Claim Forms in a sentence

  • Within ten (10) days from entry of the Preliminary Approval Order, Notice shall be provided on a website at an available URL (such as, for example, xxx.XXxxxxxxxxxxxxxxxxxXXX.xxx) which shall be obtained, administered and maintained by the Settlement Administrator and shall include the ability to file Claim Forms online, provided that such Claim Forms, if signed electronically, will be binding for purposes of applicable law and contain a statement to that effect.

  • Class Members will have until 180 calendar days from the Notice Date to submit their Claim Forms (“Claims Deadline”), which is due, adequate, and sufficient time.

  • Claim Forms submitted using the Settlement Website must be submitted by 11:59 p.m. Eastern Time on the final day of the Claim Period.

More Definitions of Claim Forms

Claim Forms mean the form eligible Class Members must complete and timely submit to receive a Settlement Payment under this Settlement Agreement. The Claim Form submitted to the Court for approval must be in the form attached as Exhibit D.
Claim Forms means the claim form included in the Settlement Notice, as defined below, for Eligible Settlement Class Members to opt-in to the litigation.
Claim Forms means the claim form submitted by a Settlement Class Member, in the form attached hereto as “Exhibit C”, to receive a Settlement Award pursuant to Section III. Each Settlement Class Member must attest under penalty of perjury that they qualify as a member of the Settlement Class, that they purchased a VIZIO Television during the Class Period, and the information supplied in the Claim Form is true and correct to the best of the Settlement Class Member’s knowledge. For a Claim to be considered valid, each Settlement Class Member must enter the following information into the Claim Form: (1) Television model number(s), (2) approximate date(s) of purchase, and (3) the place(s) of purchase (selling retailer name and state of purchase). For online purchases, the place of purchase is the Settlement Class Member’s state of residence at the time of the purchase. Additionally, for a Claim to be considered valid, each Settlement Class Member must provide “proof of purchase” concurrently with the Claim Form by submitting one (1) of the following:
Claim Forms refers collectively to the Premium Overpayment Claim Form and Identity Theft Claim Form.
Claim Forms means the Tier 2 and 3 claim forms made available to Class Members substantially in the form of Exhibits E and F hereto.
