Direct service definition

Direct service means providing services to minor or vulnerable adult when the services are rendered in the physical presence of the minor or vulnerable adult. Services include, but are not limited to: providing individual services such as counseling, mentoring, job coaching, training, job search activities, testing or providing mental health and medical services to DWS customers. See Title 62A, Utah Human Services Code, Chapter 5, Services for People with Disabilities.
Direct service means services involving face-to-face assistance to a consumer such as transporting a consumer or providing therapy.

  • The State will promptly pay the Contractor after the Contractor presents an itemized invoice for the services and the report(s) required under Item 2, Scope of Work, for the services actually performed and the State's Authorized Direct Service Staff Representative accepts the invoiced services.

  • Each disclosure and each receipt of Health Data by Subscribed User through Direct Service shall constitute a certification by Participant that Participant and Subscribed Users are complying with this Agreement.

  • CRISP shall only access Health Data for the express purpose of connecting the Participants, facilitating the delivery or receipt of the Health Data using the Direct Service on behalf of such Participants, providing telephone level help desk services, and otherwise fulfilling its obligations under the Agreement.

  • If any effective increase on or after July 1, 2015 is unacceptable to Participant, Participant will have a special right to terminate the provision of the Direct Service as of the effective date of the increase by providing written notice to CRISP within thirty (30) days of receipt of notice of the increase.
