Office Visit definition

Office Visit means: (1) an evaluation, consultation, or physical examination that is performed by a medical doctor (M.D.), doctor of osteopathy (D.O.), or a nurse practitioner (N.P.); (2) an initial psychiatric evaluation only when conducted by a provider licensed to perform that evaluation; and (3) an initial evaluation only when performed by a chiropractor or physical therapist for an Injury (limited to two per Calendar Year). The term Office Visit also includes minor surgical services that do not require the use of a surgical facility or suite, and Home Health Care services.

Note: This definition of Office Visit does not apply to any Policyholder who has elected the optional co-payment endorsement, form #’s WPMAEND1GEN (1/99) or WPMAEND1GEN80/60 (1/99). If your Policy contains that endorsement, please refer to that endorsement for the applicable definition.
Office Visit means a visit by the Insured Person, who is the patient, to the office of a Physician during which one or more of only the following 3 specific services are provided:

Examples of Office Visit in a sentence

  • Additional services, such as treatment and diagnosis of Mental/Nervous Conditions, or laboratory, x-ray, and other Diagnostic Services are not included in the Office Visit.

  • For purposes of this definition, a Medically Necessary visit by a Physician to a Homebound Participant's place of residence may be considered an Office Visit.

  • For purposes of this definition, a Medically Necessary visit by a Physician to a Homebound Insured's place of residence may be considered an Office Visit.

  • Mental Health Coverage for All Other Mental Illness: Non-emergent outpatient mental health visits when medically necessary and referred by your Primary Care Physician to a Plan Provider are provided for at the Mental Health Office Visit cost-share or Mental Health Outpatient Other Items and Services cost-share as shown in the “Health Plan Benefits and Coverage Matrix” section and described below.

  • Office Visit Copayment If your Plan has an Office Visit Copayment, this is the amount of Cost Sharing you must pay each time you have an office visit with an In-network Practitioner/Provider.

Office Visit means a medical visit or Outpatient consultation in a Physician’s office or patient’s residence. For this purpose, a Physician’s office is a medical/office building, Outpatient department of a Hospital, freestanding clinic facility, or a Hospital-based Outpatient clinic facility.
