Plan Coordinator definition

Plan Coordinator means an MHS, CSW, or MFT that provides mental health, crisis 6 intervention and case management services to those Clients who seek services in the COUNTY operated 7 outpatient programs.
Plan Coordinator means a person designated to coordinate the development of the individual’s service plan.

Order coordination for physical conversions will be scheduled at BellSouth’s discretion during normal working hours on the committed due date and Choctaw will be advised.

  • Relationship Manager means the individual who is appointed by us from time to time and notified to you in writing as your relationship manager in relation to the relevant services provided to you under these Terms and/or provided by our Associate;
